My wife brought home this form from the book study to fill out. I don't like it at all and don't want her to fill it out because I think it is an invasion of privacy and also because I'd like to be difficult and ignore the society's attempt at controlling its members.
This form was photo copied and is not an official form from the society...no S- code or any thing. It just looks like someone made it on their PC and photo copied it.
Some of the questions are interesting and one must wonder why they want to know...or maybe they are just worded funnily. Anyway, I'll reproduce it below (no scanner yet).

I thought congregations weren't supposed to compile and distribute phone lists any more of people at the hall.
I'm going to recommend to my wife not to fill it out and turn it in. Can anyone think of a good reason for my stance on this...one that an elder will buy, anyway? I'd rather they didn't know our relatives and where they live...our friends and contacts away from our town...or our email address or cell phone numbers.
