Ain't that stupid? How the hell can everyone stay in the middle???? By definition, it'd be impossible, someone has to be on the outside!!!!

"That's a fine how do you do." What the hell does that even mean.
"How do you do" is a standard greeting. So, replacing "standard greeting" into the sentence, we have "That's a fine greeting." Greetings are supposed to be cheerful and show some respect, so I've always read that phrase to mean that the person is not showing any respect or consideration. I don't use this phrase, but I don't mind it either.
