Did you notice that this thread is 10 years old? A few years ago Lady Lee left this discussion board, so it is unlikely you will hear anything from her.

Yes. I saw that the thread was super old. I wasn't really participating in the discussion. Just trying to update the links. This was the first thread that came up when I searched for this book.
From time to time I'll get a PM from someone looking for a publication. I like for there to be working links to info, just for posterity.
Googling KM's & Review Answer sheets is what brought me here (Thanks Atlantis!), So I figure I'll help out future searchers by keeping things available.
