I could see possibly asking the blood card back from someone that voluntarily disassociates and makes it plain that they want nothing further to do with the Witnesses.
However disfellowshipping is altogether different. According to the Witnesses, its purpose is to lead the person to repentance. This would include obeying all of "Jehovah's laws" including those the Witnesses believe apply to blood. Asking for the blood card back in this situation makes no sense at all
