From what I remember it's not a quiz its more of an informal study were they just go over everything you learned and see if you are happy and ready, I think they just prefer you to be honest with what you want and your answers.

I was baptised in the Name of the father ,the Son, & the Holy Spirit. BUT NOW!!! You will have to agree to be baptised in the name of Jehovah ,Jesus Christ & THE SPIRIT FILLED ORGANIZATION>>>
Because they were getting sued so much that now you cant really sue if you adhere to their baptism as any Organiation you join you go by there rules.

Just before they haul you off to be dunked at the assembly/convention these formal questions are asked:

The newest baptismal questions, from the June 1, 1985 Watchtower:

    (1) On the basis of the sacrifice of Jesus Christ, have you repented of your sins and dedicated yourself to Jehovah to do his will?

    (2) Do you understand that your dedication and baptism identify you as one of Jehovah's Witnesses in association with God's spirit-directed organization?

The questions are in the OD (Organized to Do Jehovah's Will book) (This site may be invisible on JWD www.watchtower.cc)

There are 3 sections of questions in the book, ORGANIZED TO DO JEHOVAH'S WILL.
Literally more than one hundred questions (subquestions too). They are generally covered in 3 sessions of at least
one hour each. The questions have many scriptural references instead of straight out
answers in the book, so that you will assume the doctrine being enforced is from the Bible.

Part I- Elemenary Bible Teachings (35 questions) Here's a sampling:
    1. Who is the true God?
    5. How should we view and treat God's personal name?
    6. Why is it important for us to use God's personal name in worship?
    10. Who is Jesus?
    16. What is the Kingdom of God?
    20. Who is Satan the Devil?
    25. What is the human soul? Can the soul die?
    31. What hope is there for ones who die?
    35. Why should we hold firmly to our hope in the resurrection of the dead?
Part II- Jehovah's Righteous Requirements (37 questions) Here's a sampling:

    1. What is the Christian standard for marriage?
HERE's A NEWER ONE ADDED BY THE PARANOIDS:
    9. To avoid transmitting an infectious or a potentially fatal disease to others, why should an infected person
    (a) not initiate public displays of affection, such as hugging and kissing?
    (b) attend the Cong. Book Study at the Kingdom Hall if at all possible?
    (c) not react negatively when some choose not invite him into their homes?
    (d) Why should a person who may have been exposed to an infectious disease voluntarily choose to have a
blood test before beginning a courtship?
    (e) Why should one having a communicable disease inform the presiding overseer before getting baptized?
    10. Why should Christians abstain from blood, and what does it mean to do so?
Bizarre enough that they list a Mosaic Law scripture in the references for #10, then question #11 negates it-
    11. Are Christians under the Mosaic Law and it's requirements regarding sacrifices and the sabbath?
    25. In addition to confessing his own sins, each Christian has what responsibility regarding serious wrongdoing by others
that could threaten the spiritual or moral cleanness of the congregation?
(Scripture shown is "In case a soul sins in that he has heard public cursing and he is a witness or he has seen it or has
come to know of it, if he does not report it, then he must answer for his error. - Lev. 5:1)
    29. What is the Christian's position as to the world alienated from God?
    35. What is the only religious ceremony that Christians are commanded to observe?
    37. What birthday celebrations are mentioned in the Bible? How does this affect your view of birthday celebrations?
Part III- Jehovah's Arrangement of Things (32 questions) Here's a sampling:1. In God's arrangement of things, who is the head of the married woman?
    8. Are there any circumstances under which a Christian would refuse to obey worldy rulers?
    11. Who is the head of the Christian congregation?
    12. How can you identify "the faithful and discreet slave" today?
    13. What is the Governing Body of the Christian congregation?
(Scriptures shown are "Certain men came down from Judea and began to teach the brothers: 'Unless you get
circumcised, according to the customs of Moses, you cannot be saved.' But when therre had occurred no little
dissention and disputing by Paul and Barnabas with them, they arrangded for Paul and Barnabas and some
others of them to go up to the apostles and older men in Jerusalem regarding this dispute." - Acts. 15:1,2

"As they traveled on through the cities, they would deliver to those there for observance the decrees that had
been decided upon by the apostles and older men who were in Jerusalem. Therefore, indeed, the congregations
continued to be made firm in the faith and increase in number from day to day." - Acts. 16:4,5)
    19. Why must approved works accompany our Christian faith?
    23. How can we give material support to the Kingdom work today?

Right after all the questions in the book, there are INSTRUCTIONS FOR CONGREGATION ELDERS that tell them
how to get through the questions. It mentions that they "make sure that a person to be baptized has aquired a
reasonable understanding (italics theirs) of basic Bible teachings. Additionally, they will want to ascertain whether the
prospective minister deeply appreciates the truth and demonstrates proper respect for Jehovah's organization."

There are two pages of INSTRUCTIONS FOR CONGREGATION ELDERS. The CONCLUDING DISCUSSION WITH
BAPTISM CANDIDATES section is only to be covered after the elders decide it is okay for the person to get baptized.

Also- Concluding Discussion with Baptismal Candidate (This follows Part III)

(Included here are the actual questions that will be asked at the baptism)

On the basis of the sacrifice of Jesus Christ, have you repented of your sins and dedicated yourself to Jehovah to do his will?

Do you understand that your dedication and baptism identify you as one of Jehovah's Witnesses in association with God's
    spirit-directed organization?
