Just a tip, poster Atlantis has the most complete file on WTS info/publications/etc. If he doesn't have it, he knows who does.

I have it but it is in Spanish. Might be able to use google translate if you want. And there are other BOE's concerning disaster relief you are welcome to.

Here is a zipped folder of files including the S-180 and some other BOE's and so forth. There is one file in there called "Disaster Relief Fraud" that has some scans and forms you might want.

The DC-50-E form has a grey box for the volunteer to check.

The brochure "Jehovah's Witnesses and Disaster Relief" might be a little tricky to get, because it was printed mainly for government officials. (From your link: It was especially prepared for government officials in the United States who oversee emergency management efforts.)
