I would like to obtain a copy of the form used by the elders to inform the Branch Office when a person is disfellowshiped or disassociated the S- 77.
The form needs to be the one used in 1994 preferably printed in UK.

Thanks Gentle dawn much appreciated. To be honest I have this copy as its printed in USA. I am looking for the one printed in UK. But this will do.

I would like to know when this form began to be used. I disassociated around 1981. I wonder if there is such a form in some office in New York about me! I suppose the WTS considers these forms confidential and that it is not possible for a person to get a copy of the form about his/her disfellowshipping/disassociation.
