I would like to know if anyone can tell me how long has this form been out there. Found my husbands about a year ago and my name is not on it. I made several copies for myself. One day I was feeling a little crafty so I went into his bedroom and took down the marriage license. I replaced it with a copy of his advocate form, after all why hide such a important form. He took it down once, but I put is back up. As far as I am concerned the people on that form are his wife, he can sleep with the fish.
