He's been on the run now for four years, and at 57 years of age, he isn't a young man anymore, although he does appear to be in good shape.
I think there are three possibilities for where McLean might be:
1. McLean is no longer in the United States. There are many nations that wink at sex tourism and don't really care that their children are being exploited by pedophiles. McLean could be in one of those countries, or he could be in a nearby "couldn't care less" country with barely functional law enforcement, like say, Mexico. The amount of money he may have taken with him was large, but not large enough to provide him with a James Bond Supervillain lifestyle. Sooner or later he is going to have to find a way to make a living.
2. McLean might be in an isolated, remote portion of the US living the life of a crazy desert hermit, eating low-cost grasshoppers and drinking low-cost cactus juice. It is known that he probably has the skills to do this, but long-term such a life would not be very "satisfying" to a man with his needs. His cash could last a lot longer that way, but he wouldn't even be earning interest on it if it is in a buried ammo box out in the desert somewhere.
3. McLean might be dead. He could have run off into the desert and experienced a fatal mishap or decided to off himself, or he could have left the USA and died or suicided on foreign soil. This would bring his living expenses down to about zero.

Doesn't he remodel cars for a living? That's a pretty easy cash and carry type business to do anywhere. Especially if he went to Mexico or somewhere further south of the border. As sad as it sounds, unless he murdered someone he's probably not going to be extradicted either. Or at least I can't see a big issue of extradicting him for the charges brought forth.
He's probably still a witness somewhere. If he were smart he'd be in a country that does not have extradiction. However I can't see him finding that out before travelling.
