Ok folks, I have been nagged so have worked out how to scan and upload images and here they are for you, front and back of the card, and 3 pages of the instructions on how to fill them in. Bear in mind it's UK so possibly may be different where you are.

Sorry about my wonky scanner (it honestly isn't me!) I can mess about and get them straight but time seems to be of the essence! Hope it helps. especially dont like the point about sharing files with the HLC..
